What does Reconnect Community Health Services offer?

Outpatient (OP)

Outpatient programs are for those seeking mental rehab or drug rehab, but who also stay at home every night. The main difference between outpatient treatment (OP) and intensive outpatient treatment (IOP) lies in the amount of hours the patient spends at the facility. Most of the time an outpatient program is designed for someone who has completed an inpatient stay and is looking to continue their growth in recovery. Outpatient is not meant to be the starting point, it is commonly referred to as aftercare.

Residential (inpatient) Treatment

Residential treatment programs are those that offer housing and meals in addition to substance abuse treatment. Rehab facilities that offer residential treatment allow patients to focus solely on recovery, in an environment totally separate from their lives. Some rehab centers specialize in short-term residential treatment (a few days to a week or two), while others solely provide treatment on a long-term basis (several weeks to months). Some offer both, and tailor treatment to the patient's individual requirements.

Treatment Types:

Individualized Treatment

Certain drug and alcohol rehab centers have standard treatment regimes they expect all patients to follow. Others offer individualized treatment, meaning they tailor treatment to a person’s specific background and needs. For example, a rehab facility may adjust a treatment program to take into account the type of drug or addiction from which the person suffers, their age, medical condition(s), religious beliefs, or lifestyle.

Therapies Offered:

Individual Therapy

In individual therapy, a patient meets one-on-one with a trained psychologist or counselor. Therapy is a pivotal part of effective substance abuse treatment, as it often covers root causes of addiction, including challenges faced by the patient in their social, family, and work/school life.

Group Therapy

Group therapy is any therapeutic work that happens in a group (not one-on-one). There are a number of different group therapy modalities, including support groups, experiential therapy, psycho-education, and more. Group therapy involves treatment as well as processing interaction between group members.

Reconnect Community Health Services Highlights:

The Reconnect Assertive Community Treatment Team (ACTT) is a collaborative team of mental health professionals which offers intensive support to individuals with complex mental illness, addictions and co-morbid issues. Some of the service options include nursing, behaviour therapies, case management, psychiatry and a Nurse Practitioner.
The South Toronto Link Team program is a collaborative team of mental health professionals that provides highly coordinated services to clients who are 16 years and older with urgent and complex mental, physical and emotional needs. Clients often have a history of frequent use of emergency departments, short-term crisis units and inpatient units with a poor track record of system engagement. Personality disorders, PTSD and challenging behaviours are not uncommon.
Forensic Case Management Services is a partnership with the Centre for Addiction and Mental Health (CAMH) Law and Mental Health Program, that supports individuals, 16 years and older, with serious mental illness who have been found Not Criminally Responsible (NCR) and are thus under the jurisdiction of the Ontario Review Board (ORB).
Through Supporting Clients Currently in Housing, Reconnect provides support to individuals currently in housing in partnership with Supportive Housing in Peel (SHIP), Canadian Mental Health Association (CMHA) Toronto, and Mainstay. These programs serve the Toronto Central and Central West LHINs. Housing with CMHA and Mainstay is accessed by completing an application through Access Point.
|The Mental Health and Justice Short-Term Crisis Bed program provides short stay residential support in the community to individuals who are living with serious mental illness, are involved with the judicial system and are in crisis. This program services the Greater Toronto Area (GTA).
